Purpose: The potential benefits of enhanced reablement capacity are significant. By supporting individuals in regaining independence, this increased capacity could reduce the need for hospital admissions and long-term care placements, leading to longer-term cost savings across the health and social care system. The full cost of the proposed expanded service capacity, up to a maximum of 3,560.85 hours (5% increased capacity for the full potential 5-month period), plus maximum potential incentive payments at £25,000, would be fully funded by the Lincolnshire Integrated Care Board (LICB). The proposal is time-limited to the busiest annual period of hospital admission/ discharge. The proposal supports wider partnership working between the Council and the healthcare sector, in line with national objectives. Delivery of additional hours as part of the proposal would be funded at the existing and established contracted hourly rate, using a provider who is a successful and well-regarded key delivery market partner for the Council.
