Purpose: Approval to award contracts under the Provider Selection Regime Direct Award Process C, for the delivery of NHS Health Checks from 1 April 2026 to 31 March 2027, with an option to extend for a further three years (three consecutive one-year terms) to 31 March 2030 to all Lambeth GP Practices organised into nine Primary Care Networks (PCNs) as the most suitable providers to deliver NHS Health Checks to their eligible patients. The redesigned service will continue to be based on the most relevant and up to date national guidelines (e.g. NHS Health Check best practice guidance 2019 and NHS Health Check programme Standards July 2020) and considers local insight and priorities that have been further shaped during the course of the current contract. The principal objective is to reduce the chance of a heart attack, stroke or developing some forms of dementia for Lambeth’s eligible population, providing individuals with behavioural support and, where appropriate, pharmacological treatment.

Content: To award contracts for the delivery of NHS Health Check Services from 1 April 2026 to 31 March 2030 to the following practices organised into nine Primary Care Networks (PCNs): ·       AT Medics PCN: Streatham High Practice and Edith Cavell Practice ·       Brixton and Clapham Park PCN: Clapham Park Group Practice, Hetherington Group Practice and Hetherington at the Pavilion. ·       Clapham PCN: Clapham Family Practice, Grafton Square Surgery, Dr Curran & Partners, and Sandmere Practice. ·       Croxted PCN: Paxton Green Group Practice, The Old Diary Health Centre, and Brockwell Park Surgery. ·       Fiveways PCN: Vassall Medical Centre, Akerman Medical Practice, Minet Green Health Practice, Herne Hill Road Medical Practice and The Corner Surgery. ·       HBD PCN: North Wood Group Practice, Herne Hill Group Practice, Brixton Hill Group Practice, the Deerbrook Surgery, Prentis Medical Centre, and Knights Hill Surgery. ·       North Lambeth PCN: Waterloo Health Centre, The South Lambeth Road Practice, The Vauxhall Surgery, Mawbey Group Practice, Lambeth Walk Group Practice and Hurley and Riverside Practices. ·       Stockwellbeing PCN: Binfield Road Surgery, Stockwell Group Practice, Beckette House Practice, Springfield Medical Centre, The Grantham Centre Practice. ·       Streatham PCN: Valley Road Surgery, The Exchange Surgery, Palace Road Surgery, The Streatham Hill Group Practice, Streatham Common Group Practice, and the Vale Surgery.   The total annual value of all contracts is £245,000 and total maximum value of all contracts across the duration of the contract is £980,000.
